Questionnaires that must be issued to citizens of the Republic of Latvia and persons whose permanent residence is Latvia (holders of permanent and temporary residence permit and long - stay "D" visa, provided that the issuer is the Republic of Latvia). The questionnaires are in three languages.
The respective carrier is responsible for handing over completed persons supporting document to the State Border Guard in Latvia.

Questionnaires that must be submitted by Lithuanian and Estonian nationals and persons residing in Lithuania or Estonia (holders of permanent and temporary residence permit and long-stay "D" visa, provided that the issuer is Republic of Lithuania or Republic of Estonia) who return to Lithuania in transit through Latvia or Estonia.
After completion, questionnaires must be handed to the State Border Guard.
